Ice-Pick Lodge has shipped the Great October Update for Pathologic 3 on Steam, and the headline change is a pair of difficulty presets built around the game’s original Normal mode. The update also adds a photo mode, separates subtitle and voice-over language options, reworks the Shabnak encounter, and optimizes performance for weaker CPUs and the Steam Deck. The full patch notes run ten pages and, the studio warns, contain story spoilers.
The two new presets sit on either side of Normal, which stays exactly as it was at launch. Easy reduces the pressure of the game’s systems so players can follow the story without fighting the clock as hard. Hard demands tighter resource management and a better read of the mechanics. In the patch notes, difficulty adjusts “the amount of amalgam available” and “how strongly mania and apathy affect the Bachelor.” Amalgam is Pathologic 3’s core resource, and mania and apathy are the mental states that shape how the Bachelor reads the Town. The difficulty selection screen explains each parameter before a run starts.
The subtitle change is aimed at international players. Before the update, voice-over and subtitle languages were tied together. Now a player can keep the original Russian voice acting while reading English subtitles, which the studio frames as a request from the community.
Photo mode lets players adjust scene, weather, and lighting, add stickers, and frame screenshots. Ice-Pick Lodge says the Pathologic community has long shown artistic talent and hopes the tool lets players see the Town from a different angle.
One bug is deliberately staying. The developer letter names the invisible apple tree as the game’s most famous bug and says it is not being fixed. The studio admits it hated the tree at times, but the annoyance has turned into affection, and the tree now reads as accidental art rather than a broken asset. Quest, dialogue, level-design, balance, and UI fixes do arrive, including corrections to Isidor’s Funeral on Day 2, the Musical Interlude decree, Saving Andrey on Day 3, and the Prison Break and Crucible sequences on Days 5 and 6.
Ice-Pick Lodge did not publish console-specific patch notes in the two Steam posts, so Steam is the confirmed platform for this update, and it is live there now.
